Proceed to the Guidance Office and request a Personal Data Sheet (PDS).
Fill out the PDS and submit it along with the following:
Two (2) 1×1 ID pictures
One (1) long brown envelope
Original copy of your Certificate of Good Moral Character
The Guidance Office will issue your Admission Slip.
๐๐ฉ๐๐ฅ 2:
Go to the Clinic and present your Admission Slip for medical clearance.
๐๐ฉ๐๐ฅ 3:
Proceed to the Registrarโs Office and submit the following:
PSA-authenticated Birth Certificate (original copy)
Form 138 or TOR (original copy)
The Registrar will process and issue your Certificate of Registration (COR).
Please ensure that you proceed with the enrollment only if you have ๐๐จ๐ฆ๐ฉ๐ฅ๐๐ญ๐ ๐๐ซ๐๐๐๐ง๐ญ๐ข๐๐ฅ๐ฌ, as incomplete requirements will not be entertained. Kindly accomplish all enrollment steps within the enrollment period. For any inquiries, feel free to send us a private message.
